Please, Please, for the well being of mom, get an ultrasound asap... I had a 
client (which I posted on previously) who's blocked duct became an abcess.  I 
wouldn't wish the surgery she had to remove it on anyone.  The incision was 
too close to the aerola and she had a heck of a time with milk issues either 
leaking out of the open wound or unable to put baby to breast b/c of pain.  
What you are describing sounds like its becoming more of an abcess than just 
a blocked duct.  Has she started on antibiotics yet?  I hope she gets the help 
she needs and gets this resolved before any kind of I&D would be indicated.  
(Incision and drainage).
