I wonder if people are combining pumping because baby won't nurse
with pumping when baby can't (i.e. in NICU and NPO).  Pumping when
baby won't be nursing for some time because of a medical condition
such as prematurity or respiratory distress would IMHO start a lot
sooner than pumping because of difficulty getting baby to latch and
nurse.  I like to have NICU moms start by at least 8 hours. That's
been a challenge to get RNs to comply who in the past would often
wait until the next day!  In time once we get compliance with
starting by 8 hours we may consider moving it up earlier or at least
offer moms the opportunity earlier.  A mom in this situation feels
"left out" of her newborn's care and pumping gives her an important
role to play even if baby has a lot of "technological care" going
on.
