The first thing that jumps out at me, in spite of the breast reduction and the unusual oral structure is the possibility that this mom may have an OAMER and/or very large supply. Many of the behaviours you describe could eaily be caused by a baby trying to compensate for oral structure, but could also be a baby trying to survive a very rapid flow and supply beyond his control. Has this been ruled out? I have seen babies with very high palates behave as you describe and the solution has been to get more breast in the mouth to create a better seal. If the flow or supply is too much to handle, the baby would certainly resist such an effort.  It certainly would be consistent w/  the very rapid weight gain in spite of poor latch. Maybe this baby cannot make a good seal, or maybe he just doesn't want to. It could also explain the gassiness and infrequent stooling since the fat content of the feeds would be quite low (proportionally), as the baby is never really "emptying" the breast.
