Long ago I decided NOT to create another form to use...that I would  probably
forget to use anyway. For the last six or seven years, I've used a  small
wire bound note book for phone calls. It's easy to just write the date and  time
(especially from voicemails) name, date of birth, birth weight etc as you
talk or listen to the message. If it was a call back with no answer, I'd write
LM (left message) with the time and date or "no machine, talked to dad or
whatever".  (Sometimes I'd use a different color pen from the original  message.)
It's easy to look back for previous messages and calls received. When  working
with a partner, it was not difficult to go back and read  the notes that she
wrote. If I listened to voicemail from another phone and  scribbled on a piece
of paper, I made it a point to enter the information in the  notebook
immediately upon my return to the office and then tossed the scrap.  Just remember to
date every page.
