 First, I would want to know how long "only gained a pound since"... when?  Of course, she could offer the breast more often, but a 6 month old may not be willing.  May take some expressed milk in a cup.  May take some avocado..... that's a fatty food.  May manage to get a spoonful of olive oil into some food.  Mom may raise calories in her milk by taking Omega 3 in her diet or supplements.
I have seen several 6 to 9 month old babies in the last few months who were put through invasive testing because they "had not gained enough".  They were so traumatized and so were their moms.  Yet the child, lean for sure, did not look unhealthy to me.  Alert, active, and usually the mom was slight, but not always.  Are we a society so geared to big weight gains, that the other end of the curve looks bad?  We are the most obese nation.
