I think antiviral prophylaxis is not necessary if it has been awhile since her outbreak. If the outbreak is very recent, and the birth is imminent, then I think prophylaxis is a good suggestion. I wasn't entirely clear if mother is still having skin breakdown while nursing the toddler, or if she is just worried that she will have skin breakdown with the new baby. Mother should practice good latch technique from the very first feeding, getting help if needed right away, with the new baby. If mother is  having a long run of skin breakdown then this needs to be investigated further (Paget's disease of the nipple is a scary possibility with a nipple that won't heal). Mother can also do some things that will boost her immune system like good nutrtion, fresh air, sunlight, rest and de-stress; maybe some vitamin C or other "supplements" that could help.
