Tanner's breast stages are based on English women.  Many other populations
in the world (including African) do not ever "progress" to stage 5, in that
the nipple and areola continue to form a "mound" atop the underlying breast
tissue.  Yet these "stage 4" breasts belong to women who have been pregnant
and lactating most of their adult lives.  You cannot assess the internal
structural development of the breast merely by observing them from the
outside.
