Yes, FIL and pressure feedback both work on the individual lactocyte. 
When the lactocyte and it's surrounding lobule become overfull, the 
lactocyte's integrin lets go of the basement membrane, triggering 
apoptosis of that cell. It can happen pretty quickly in areas of the 
breast with no outlet. Mom may be pretty uncomfortable in the affected 
area for a few days, but ice and analgesics should make it manageable.
