This is on letterhead...  I rarely get moms who balk at it, and if they
want to specifically exclude uses of their photos, I accept that and
have them just write the exception on the form.
For instance, I have a lovely sequence of an asymmetrical latch-
closeups of baby at the breast with mouth closed, baby opening huge, and
baby  latched.  The mom did not want me to ever post those photos on the
internet, so I have agreed to that, but I do use them in many of my
presentations.
