I would think as health care professionals we would have the same guidelines
as any other HCP. and pain in the butt that it is, begin to use the longer
form.

We use a two sided sheet for A & B with the whole thing simplified.  Parents
sign on both sides of sheet.  The whole HIPAA blather is available in a
reduced print size pamphlet, but most people decline it.

I'll be away for a few days, but will bring a copy home next week to share
if anyone wants the simplified version.
