Hi, Connie.

Hard to say based on a brief written description only, but I wonder if the
size/shape of her nipples/areola/breasts have changed since her first child?
And/or, perhaps chronic pumping has caused the nipples to swell and become
enlarged, therefore not fitting well into the flange?

It's important to determine if the sores she is experiencing are due to the
skin being 'stretched', or because of friction, or if they are from pressure
points on the flange. Type of wound can help determine this, but I am not
an expert in that; maybe someone else can help with that.

I did wonder if maybe a different SHAPED flange might work for this mom?
These ones are *oval* not round, like regular flanges. Might help?
