i generally follow protocol for induced lactation and
BFAR......domperidone,  More Milk tincture, Goat's Rue tincture.
Little to no focus on pumping if baby latches and nurses well - unless
mom is interested and well supported.  Otherwise I focus on
supplementation at the breast using a lactaid not an sns.

 i do encourage them that with a subsequent pregnancy there will
likely be more tissue development and more milk - that seems to be the
anecdotal case.  I've seen *significant* changes between baby 1 and
baby 2.  the most significant was a mom who supplemented about 50% of
nutrition the first time and just 2-3 ounces with baby 2.

I keep my mouth shut.  I've seen it more than a handful of times and
only one time was it linked with insufficient supply.  I've seen the
tubular shape, great disparity in size, wide space between breasts  in
other moms who ended up having *no* issues* with supply and have seen
other moms who had no physical symptoms but definitely had primary
supply issues of unidentified origin.

So I say nothing but keep in close contact.
