The typical OB only deals with problems waist down :-)  Do you have a breast 
specialist dr in your area?  .  That's who a mom needs to go to with any 
sort of breast problem. We are lucky to have several in South Jersey who 
know how to deal with the lactating breast.  If you have a breast dr, who 
doesn't know the lactating breast very well,  maybe you could tutor him/her 
with relevant ideas for treatment.  Pat in SNJ
