It is possible that mother is allergic to nystatin or some component within.
It seems her symptoms got worse when she and baby started treatment with
this. I am thinking perhaps she had allergic reaction and then staph
infection superimposed on broken skin.
If at all possible, I think she should see a dermatologist. Or, if not,
could she see a nurse practitioner or other primary who would use a sliding
scale for payment.
In my experience with a vaginal yeast infection, the relief is almost
immediate when an antifungal cream is used. Therefore, I do not think she
has thrush because many treatments have been  used.
I would recommend to her (in addition to seeing a doctor), these self-care
measures:
Mild soap and water cleansing, rinse well, pat dry gently with a very clean
cloth. Expose to air and light most of the day. Wash any garments that will
contact the breast in a mild soap and rinse x2 in the wash. Do not use dryer
sheets or other fabric softeners. I'm not sure I would use vinegar in the
wash. Scrupulous hand washing and washing of pump parts. Sterilizing daily
of pump parts.
Mother potentially needs a systemic antibiotic, and topical antibiotic.
Probably a topical cortisone. I would want a doctor or NP to see her though.
I hope she finds relief soon.
