Regarding the non-profit situation and pump rental:
Suggest to your director that you just charge what the monthly rental fee is
(cost) and only charge the wholesale cost of the kit. I think once you
suggest this....they may realize that your time is worth something, and time
will be needed to do the paper work, demo the pump, counsel the mom and so
on. She would not expect a staff nurse to give care for free...so why should
you? Another excellent point is that this is a SERVICE and good customer
service is essential in this day and age. Even HMO and PPO clients have
choices, and some clients/patients may choose to pay out of pocket to get the
"best." Keep track of the number of calls you get asking to rent a pump, keep
track of the number of women who go home in need of a good rental pump that
have to go elsewhere and give someone else the money your facility could have
received.  Another thought that just occured to me is the mark-up to the
patient on everything used in patient care - drugs, supplies and so on. Isn't
that a "profit?" Or is it used for overhead and other costs? Capital
purchases such as other equipment? Boy, that non-profit rationale is FULL of
holes.
