I feel like I see this with stressed babies quite a bit. Although it's  
possible it's reflux, since tongue tie and reflux often manifest together, and 
I  see it starting up anywhere between 2 and 6 weeks, I also wonder about 
this:  many moms I see are so desperate "to breastfeed" that they will not 
watch their  baby for cues that they are struggling mightily trying to get 
enough to eat at  breast, and just keep pushing the baby to keep trying, hour 
after hour or day  after day,  "snacking" as this mom describes. I don't call 
this snacking,  because that's a small amount in a person needing a small 
amount. Good  snacks satisfy.  I call this struggling, eating ineffectively, 
and not  being able to get done, to get what's needed. These babies are 
stressed. Some  will take it for a while, some with fight and start wailing 
immediately. Over  the course of the feed, day or days, the baby will become more 
and more  agitated, until they just fall apart ( or shut down). Moms think, 
or are  told, by friends and their MDs that if they  can just  MAKE the  
baby do it, just KEEP PUSHING; they undersupplement, by amount or by  
delaying, or stop supplementing completely. Strong, healthy  babies who are fairly 
mellow otherwise, or even weak babies with attentive  moms especially ones 
with very attentive moms who carry them or hold them  constantly are getting 
plenty of homeostatic support and sensory nourishment and  they will often 
put up with a lot, so mom keeps pushing, delaying or withholding  thinking 
that will just "help them go all the way" to comfortable, effective  milk 
transfer. But with tongue tie ( or the baby I saw yesterday, 7 weeks and  VERY 
ineffective tongue movements/strength and a neck and jaw  
asymmetry/tightness), the baby eventually has to make their needs known, and  show that they've 
just tried too hard for too long and have a disorganized,  screaming fit ( 
as well they should.) I don't know if that is what you are  seeing, but it 
makes sense since it happens more when you are there and mom is  likely to 
fall into a pattern you might help her avoid if you were there.  ANyway, just 
my thoughts.
