My first thought with this description is that the latch needs to be
improved. I would try reverse pressure softening, then work to get a very
asymmetrical latch with baby's head tipped back, nipple pointing to nose,
etc., and perhaps try some different positions. That will help the bottom
lip curl out as it should and the tongue fall down. When I hear the
combination of sore nipples, poor weight gain & engorgement - the first
thing I think of is a latch problem and I would work to fix that before
starting on anything else.
