a compounding pharmacy can make this for you in the US...I just prescribed
some yesterday!

You need to ask them to make 40 Units/ml.  The IV preparation is
10U/ml...which is sometimes strong enough and sometimes not. I usually ask
for 10 ml total in the nasal spray bottle.
