Diana,

I have had a similar experience.  I worked with a mother who had a low milk supply with her baby. Her appearance was so masculine that I thought it was a man playing a creepy trick on me when I walked in. This mother was very dedicated to pumping in addition to breastfeeding, but she worked in a typically masculine profession which was not at all amenable to regular pumping.  She was never able to produce more than about 1/2 oz of milk at a time.  Her breasts looked small but otherwise normal.  We discussed hormonal testing, but she decided to just breastfeed and supplement heavily since pumping at work was so difficult anyway.  We lost contact after she experienced the total loss of her home due to a tornado.  I could never find a comfortable way to really address my concerns.
