Hi All, 

 

I have a mom who has been exclusively pumping for her baby (by choice) since
birth (5/26/06).  She had a fairly uneventful pumping routine until one bout
of Mastitis (Rt breast) at 2 months (7/27/06).  A couple months later she
developed a 'tear' on the face of the nipple on the right breast.  No
symptoms of thrush - saw Dr - no help.  She was left with a 'purple' mark
where the tear was.  Now she is presenting with another 'tear' much higher
up on the breast.  Pumping is very painful for her (Using Medela Classic).
She states there is no redness, no shiny-ness, no itching or burning -
nothing that would suggest possible thrush - but does have sharp stabbing
pain in the nipple.  One last thing:  mom has 7 kids and a very busy
household and does not wash her parts after each pumping.  We talked about
how that could be contributing to her issues and we'll be getting her extra
parts tomorrow as well as some of the Medela wipes to help her out a bit.

 

I told her get to the Dr ASAP and get this cultured.  When the nipple had
it's tear in September mom reports that it oozed an amber like liquid.  I
told her two possible options: thrush or low-level bacterial infection.
I'll be seeing her in the morning to take a close up look - but she did send
a picture (that I have permission to share) along with the following history
written up by the mom herself.  Any input, thoughts, ideas greatly
appreciated.

To Whom It May Concern:

 

This note is to inform anyone who is concerned, that Jaye Simpson has my
permission to send the attached picture to any IBCLC she would like to, to
discuss the tear on my nipple.

 

History is as follows:

 

5/26/06 - birth by c-section, pumped twice that day

5/27/06 - pumped 4 times

5/28/06 - pumped 6 times

5/29/06 - started every 2 hours, around the clock

7/27/06 - antibiotics for mastitis

8/26/06 - dropped night time pumps - pumped every 2-3 hours 7am-11pm

9/21/06 - saw GYN for annual - showed him tear on nipple

 

sometime during the 2nd or 3rd week in Oct (I don't remember), I pumped
every 4-5 hours to try to get the tear to heal

 

11/04/06 - noticed pain in right nipple - sharp stabbing, pinching

11/05/06 - evening, noticed tear

11/06/06 - called Jaye

 

will try to get into GYN on 11/07/06 - no guarantees - it's with the VA

will see Jaye about 1030am on 11/07/06
