Hi Moe,
You ask about a mom with a 1.5 month old baby and 2 wks ago
experienced a plugged duct on the right breast. Since then it seems
the baby was fussy on the right side, mother felt that her milk supply
had decreased, started using some formula, and began pumping to assess
her supply. That's what I gather from your post.
Is the plugged duct resolved now? If not I would suggest warmth and
gentle massage, and frequent feeding. If the plug is resolved, then it
seems mother might have gotten a little off track of "natural bf." So
I would steer her back to bf on cue, eliminating the formula as
quickly as possible, and I would evaluate the latch and bf mechanics.
Sometimes the latch has been less than effective from the start, and
that will lead to a diminished supply over time. Sometimes as the baby
gets older, the basics of a good latch and a close positioning get
forgotten and baby is twisting or pulling on breast, leading to poor
drainage. I know you said mom's history is negative, but is she on any
meds including birth control products? Also I know pediatricians go by
the rule of thumb 'regain birthweight by 2 wks' and this baby regained
by 13 days. But to me that's a slower than usual wt gain (if well,
term baby) so I would want to explore the basic bf mechanics and
practices.
