I am counseling a mom with yeast on her nipples for 2 months now. What are
your prof. opinions with the following case

Baby 5 months
At birth- was in nicu
Mom was treated with antibiotics

Yeast developed 2 months ago. Shooting breast pain, burning nipples
Treated with diflucan 100mg X 14 days and nystatin oint. Peds would not treat
asymptomatic baby.

Yeast cleared, mom felt better until 3 days after treatment finished: all
symptoms returned. OB gave her another round of treatment. Peds still would
not treat baby.

Mom got good relief again for two weeks and then same relapse. OB put mom
back on diflucan and tried gentian violet this time, 1% sol to moms nipples 1
time a day. Mom stated her nipples felt more dry with GV. Baby finally got
treated after I called Peds and spoke to him. Baby on Nystatin Susp.
Her nipples are more itchy than before and she is at her wits end. Did not
want to quit but is considereing it.
She also has used hydro cortisone topical. Has added acidophilus to diet, cut
out dairy and is vigilant about all other ways to cut out yeast recurrence.
Her OB is sending her to a dermatologist tomorrow because he basically feels
we may not be diagnosing it correctly.
What am I missing here?
 I was surprised that she did not get relief with GV which is making me
wonder if this is yeast.
