Sorry for my ignorance but what is the australian hold?

It is not unusual to see asymmetric breasts, if milk is only removed from
one breast. If mom is only feeding on left side AND pumping the left side
also, that could be your explaination for the engorgement. That breast is
working overtime. I would suggest feeding on left side until baby has her
fill. Not limiting the time unless soreness develops. And to pump only the
right side to keep the milk production going. It may help the nipple to
protrude better. Continue to try to put the baby on the right side few
times a day.
You might also want to look at other causes of nipple soreness such as
thrush, tongue-tie, Raynaud's, not releasing the latch while taking the
baby off etc. Hope this helps!
