I wrote a whole book on it for Hale Publishing in 2008 - "Best Medicine: Human Milk in the NICU" with Jane Morton and Jae Kim.  Don't think hard copy is available anymore as haven't had time to update.  May still be available on line.  Otherwise see www.cpqcc.org, click on quality improvement, then toolkits, then Improving Nutrition for the VLBW infant.  Also UCSD's SPIN Program is excellent (online).
