Sometimes I think the pain is from a large baby sucking very hard and with
a flow less than optimal.

Perhaps with prior breast surgery she has a max production that is not
enough for this particular LGA infant.

A baby with birthweight 9#14 needs lots of milk.

At this point, seems he is being underfed.

To improve his intake, breast compression, supplement with all ebm and
formula if needed. Consider a nursing supplementer at breast.

Frequent weight checks.

Hospital grade multi-user pump rental.

Galactagogues - give realistic expectations - these don't work miracles - I
typically suggest food/herbal and not the prescription ones.
