Hi all!  I am working with a mom who is working on breastfeeding her
third child.  She has successfully breastfed one child previously, for
18 months.  She reports that the baby "flicks" his tongue and "flutters"
his jaw when on the breast and that her nipples are so sore that even
light pressure is excrutiating.  She describes the pain as feeling like
"pins".  Baby is now 18 days old, has regained to birth weight, is
having yellow bm's (smears) with every feed.  She has been double
electric pumping with a PIS and feeding to baby with an AVENT bottle,
slow flow. Mom has used a combo. of nystation and triamcinalone to her
nipples (given by pediatrician) for one week with no improvement, and
the baby has been on oral nystatin and nystatin to his diaper rash.  I
initially agreed that it sounded like yeast, but the pain has gotten no
better with topical treatment. Maybe it's ductal.  Maybe its a latch
issue.  So.....tomorrow I'll observe the latch but - if that looks good,
and oral/suck exams look good - is diflucan in order or am I
overthinking yeast?  No h/o antibiotics or chronic yeast infections.
She says she knows her ob-gyn will NEVER give her diflucan without
SEEING yeast with his own eyes.    Baby has a "raw" diaper rash - bleeds
with diaper changes. Also - how does anyone teach suck training to
moms?  Any advice?  thanks - this one's got me sweatin'......
