I hope you post more often, isn't it great to get such great feedback so
quickly? You have gotten some great replies already, so I'll  just add a
little something. You said mom's breasts are large and her nipples almost
flat. This can be quite a challenge. Is the baby she is adopting full term,
near term or premature? The reason I ask this is to go back to basics and
get good positioning with support of mama, baby, and breast. Maybe a nursing
pillow would help or have you tried various positions to see what works
best?
Perhaps a hand pump could be used, mom can also use her own hands, to evert
the nipple prior to latching or prior to applying a shield. Even though mom
has not been pregnant or lactating, perhaps using the technique reverse
pressure softening (RPS) prior to latching would evert her nipple more.
The other thing is that many moms prefer the LactAid Nursing Trainer over
the SNS or even a homemade nursing supplementer using a #5French feeding
tube that is 36 inches long and threaded through a cut nipple on a bottle of
formula. Of course these only work once the baby is latching, so that is the
first step to get accomplished.
