At our hospital, University Medical Center, we have a consent form for any supplementation to breastfed newborn babes.  They have had this consent form for over 10 years.  There always has been controversy on its use with the nursing staff.  Some say mothers sign it without really reading it. Other says it helps validate what is being taught. Most says its a great reminder for staff. If we have mothers who insist on supplementing, we supplement on the breast with a SNS as much as possible.    Giving supplementation to any breastfed babe needs to be done as informed consent.  This consent form satisfies that need.  With education and informed consent being a highlight for JACHO requirements, somewhere along the line a mother needs to be informed of the consequences of "topping off" a babe.
