Okay - I searched the archives.

The first reference I could find was a quip by Lisa Jones of Wellington,
FL, on 18 Feb 2000 about "classic oro-boobular disproportion", as though
she used the phrase frequently. Subject line was    Re: no latch
I searched by her subject line to see if it had been mentioned earlier in
that thread, but I didn't see it.
Jan B. used "oro-nippular disproportion" on 23 Mar 2000 and referenced Jean
C.
Jean C. replied & denied it. So Jan B. may be the first with the revised
version. Jan B. later used "oro-bubular disproportion" on 8 Sept 2002.

So I think credit goes to Lisa Jones, unless she says someone else said it
before she did.  :-)
