Welcome to lactnet.
This part is significant to me: "The mom arrived at hospital about hr before
the birth - no time for interventions. Breastfeeding was initiated but the
baby did not do well and a stomach washout was done. Bf did not improve."
This tells me it might be a baby problem which led to low milk supply due to
ineffective breast drainage. Of course it may be a maternal problem (check
mom's history like breast growth, engorgement, and also breast appearance)
but I don't think that would manifest at first few feeds. Next, "a loss of
12.9%" then a nice gain with "baby had been receiving 50-75ml ABM 3 hourly
after attempting to first breastfeed." This would be 400ml to 600 ml per
day, could be half or more of his intake easily.
So....I would go to the basics, as it appears you are doing 1. feed the baby
as is being done, 2. maximize the mother's milk supply, 3. keep them bf. I
would investigate infant problems like tongue-tie, high palate, low tone,
birth injury, maybe mom's nipples very large, whatever detective work to be
done. Maybe craniosacral therapy or other tx can be done. Check our archives
