I think the best way to understand is to ask the mother herself. Given
such a vehement statement, you could ask--- "You feel very uncomfortable
about breastfeeding; can you tell me about this?"  or some such
open-ended question, in an open tone. If she says something like, "I
just think it's gross," then you can say, "it sounds like you've had a
bad experience...."  etc.

If the issue is sexual abuse, the mother may or may not be aware of it
and probably will not be forthcoming. However, listening and empathy may
open the doors for some productive dialogue. At the very least, it may
help her be aware of the source of her reactions. I always try to keep
Rule #1 in mind-- mothers must feel heard and cared about before they
can open up and learn and change.
