She has tried several thrush remedies, including a course of =

This sounds like thrush to me. It could be she's dealing with Raynaud's, but
the deep stabbing, shooting pains unrelated to nursing sounds like she's
dealing with thrush on top of it. It would make sense that Dr. Newman's APNO
would lessen the pain, but not totally resolve it, since the shooting pains
would suggest the thrush is in the milk ducts, and topical creams aren't
going to do much for that.

If she's done the Diflucan for the full course to no avail, I'd suggest
going the herbal route. The great thing about herbs is that you're not going
to find resistent yeast the way you would with Nystatin or even Diflucan.

A good herbal regimen would be tincture of black walnut, and tincture of
goldenseal or Oregon grape root, and grapefruit seed extract. I've had great
success with this herbal regimen used by moms with cases of persistent
thrush. The key is using tinctures, rather than capsules, which are not
nearly as strong and effective.
