Unfortunately it has happened in the San Francisco Bay Area (California).
The baby needed a liver transplant.  There was of course a law suit and the
doctor I worked for at the time was asked to be an expert witness for both
sides!  Of course, he couldn't do it.

I am amazed at how little adults know about taking and giving medications.
Maybe they don't want to take the responsibility or just have never been
taught how to read labels and listen to instructions.
