Kathleen, does she have much dairy in her diet?  I've
often seen a reduction in plugs when mom
cuts/eliminates dairy.  One mom I worked with
(recurrent plugs), an MD, self-prescribed cranberry
juice pills (after my suggestion of lecithin), which I
thought was interesting.  

Can't help but think there's still an impact from
separation / bf frequency issues though I understand
she's nursing "religiously!"  These issues might not
be "fixable" though it helps to keep addressing issues
like stress (being on call perhaps & what that means
with baby) and nursing patterns (inc occasions in
which breasts are not fully emptied).
