This has probably all been covered before (I'm not feeling well and barely
keeping up around here this week) but the major points, in addition to what
Cathy Genna already posted, I stress with a nipple-confused dyad are:

Tongue position -- make sure baby's tongue is forward and down when feeding
from the bottle.  There are some good tongue exercises outlined in the
Breastfeeding Answer Book.

Tongue position can only be optimal if the bottle is not flowing like Niagra
Falls...make sure they are using a bottle that makes the baby work so his
tongue isn't blocking a too-fast flow and has to "massage" milk from the
bottle nipple.  As well, the Kassing "paced bottle feeding" technique can be
very helpful as well, because it makes baby set the pace of the feeding and
does not allow too much milk to enter baby's mouth at once (making correct
tongue position a lot easier and more likely).

I'm sorry if this was already suggested and I'm wasting space!
