Beth,
I had a case similar to this a while back. In this case it was the left
breast too, The mother had not been able to obtain even a drop of milk
from the left breast just like with  your clients case. Upon further
assessment I palpated the nipple approx 1/2 inch  below the skin
surface. There was no way I could tell if this nipple even had nipple
pores because I was unable to evert the nipple and examine it. My client
too was much engorged but we were unable to extract the milk. This
mother's rt nipple was inverted but not as severe. Her rt nipple was
visible but the baby was so angry and upset and  this made the mother
upset so it was difficult getting the baby to latch. Long story short
she gave up breast feeding.

Subject: inverted nipple and no milk

Hello,

I am an IBCLC in a hospital setting and am working with a mom I am
stumped
by.  Mom is a 20 year old primip who has given me permission to post
about
her situation. Babe was born on 7/9, breastfed well twice in the first
couple hours of life.  After that, babe started to arch away from the
breast, crying and refusing the breast for the next two days.  On Monday
morning, 7/12, I was able to latch her to the right breast which has a
slightly inverted nipple upon compression.  I tried to latch her to the
left breast which has a visibly inverted nipple which inverts even more
upon compression (but not at all the worst inverted nipple I have seen).
This latch was unsuccessful. Babe latched on for about 1 minute and then
arched back, very angry and crying. I was also unable to hand express
any
colostrum from the left breast.  I was able to hand express easily from
the
right breast and babe did latch again to right breast and fed well.  Mom
was pumping every 2 hours when she left here on Monday and was finger
feeding babe if she was unable to latch her on.

Mom came to see me today, 7/14. Babe has been latching to right breast.
Mom is still pumping left breast as babe is not latching, but has not
obtained 1 drop of milk from it.  Her left breast was very full but not
engorged.  We massaged the left breast and attempted hand expression and
did not obtain 1 drop...nothing.  Mom reports having tried hot showers,
massage, hot compresses at home and obtaining nothing. Again, hand
expression was quite easy on the right side.

Any ideas?  Why can we get nothing from this left breast and what can we
do
about it?  I am perplexed.
