Hello Aria,

Does the baby also spit up breastmilk when fed from a bottle? My question is: is it the milk or the delivery method that is causing the spit-up?

If it is the delivery method (I've seen this a lot and everyone involved is shocked to discover the baby handles mom's milk fine from the bottle), the mother could have an overactive milk ejection reflex (let down) or oversupply, which is putting stress on the baby. Both of these issues can be addressed so mom and baby can breastfeed (rather than breastmilk feed) comfortably. 

I guess I'm also interested in how much spit-up ... I suppose it's enough that the mother is concerned. 

Keep the baby's pediatrician involved in case GERD or other medical issues are suspected -- but I'm betting baby tolerates his mother's milk just fine and there's a need to tweak things in the mother. There could also be issues of gut health in the mother that need to be addressed. 

I always enjoy a good reminder that when we support breastfeeding, we support a DYAD of baby and mother; when one has an issue, always evaluate both for the cause.
