Rob,
My suggestion would be to refer to a PT, OT, Speech language pathologist
or Neurodevelopmental therapist. It is not a good reason to quit
breastfeeding however as I'm sure you know. The patient is lucky you are
her ped!  There is much that we don't know about this topic and when out
of my scope of practice, I always refer. If this is causing pain, often
one or two adjustments from a knowledgeable practioner will show
improvement. I don't believe in recipes for solving suck problems, no
matter what any book says. As helpful as it is, the Breastfeeding Answer
Book is a collection of information and not a clinical manual for
practice. It offers suggestions based on research and the experiences of
others so don't be disappointed that the technique didn't work. We don't
know all the ways to solve all the problems all the time!
