As Pat pointed out, breast milk is the least harmful ( if at all) in the rare 
case whewre a TEF exists.  As I understand it, if one exists, the risk is for 
any fluid ingested to get into the lungs.  Formula can be very irrating to lung 
tissue.  since sterile water is less so, it made sense, when most babies were 
artificially fed, for water to be given first.  However, since breastmilk is a 
"phisiological fluid' and the least likely to do any harm, perhaps the policy 
should be that ALL babies should receive sips of breastmilk before anything else 
if they want to rule out TEF!
