Cathy Genna gives some good advice re a strong pump and super pumping. The
delayed initiation of bf (1 wk delay), the trauma (car wreck) and the
continued mother/baby separation (mom feeds 2x/day) have and continue to
contribute to suboptimal lactation performance. If at all possible, have mom
do as much kangaroo care as she can. When she does visit, is it a fairly
short visit and baby is returned to incubator right after the feed? I find
this common in many nicu environments, but it would be best to have a long
kangaroo care session.

Also using a supplementer for more breast time might help. I would explore
the meds for possible effect on lactation in some direct or indirect way (as
Cathy suggests possible diuretics are having an effect), and explore use of
herbs. Perhaps a pharmacist can be consulted to help with the med/herb
situation.
