I have a question to post in hopes some of you have come accross this in 
your practice.
I had been off mail and work for 6 weeks recovering from total hip 
replacement. One of my collegues has been working with a mom regarding 
plugged ducts. Over the past six weeks this mom has had mastitis, plugged 
ducts and thrush. I suggested to my collegue that she refer her to her 
physcian for u/s of the breast to rule out galactocele. In which she did.
She also started taking lecithin. She called me today, my first day back 
to work to report that she feels the lecithin has been helping not 
plugging recently. Is still being treated for thrush and describes a 
string of peas down the breast. She was referred to a surgeon that of 
course wanted her to stop breastfeeding before being able to treat her 
stating because of the milk filled ducts is hard to visualize. I have set 
her up with a more sensitive u/s out of town and am going to be able to 
see tomorrow if there is a surgeon in that area experienced with lactating 
women. She will be coming in to see me so that I can assess the breast.
She has a urethral sensitivity I apologize for not being able to pull the 
correct term out of my tired brain at this point. She is taking lecithin 
one tablespoon four times daily. She c/o urinary frequency and discomfort 
but has ruled out uti. The weather here has been extremely hot and humid 
and I suggested she make sure her fluid intake is good as well as frequent 
emptying of her bladder. States she could do better with fluids and not 
waiting so long to urinate. Sorry for the length of this post. Look
