Ditto that stomach effect from someone who took iron once.
If the desired effect is to avoid having lactoferrin mess with the iron supplement, then one might think it necessary to avoid breastfeeding for 4 hours before AND after being given the drops - to allow all of the last feed to clear, then to allow the iron to clear, before feeding again. NOT!
Didn't I read that the natural iron in Mom's milk is at a predictable level, even when she is anemic herself?
So if baby is anemic, it originated during the pregnancy. So it should get better, or at least not get worse, during breastfeeding?

"snack" or a meal.
Besides, it seems that iron on an empty stomach would be a wonderful way
to cause excruciating
stomach pains in these poor babies. (Think about how you feel when you
take a PNV or iron on an empty stomach). 
We dietitians tell people to take vitamins with food to reduce the
nausea factor and the stomach pain.
I think most moms have common sense will just "feed their babies" and
give the vitamin or iron drops
"when they remember". 

Just my experience.
