I would perhaps see if the nipples are different sizes or the breasts are assymetrical and perhaps a different flange size would work. 
I also suggest that any mother who is pumping exclusively, and especially any mother who is using some formula for the feedings, should be using a hospital-grade pump if at all possible (as opposed to a "pump in style" pump). My interpretation is that the pump in style and other "working mom" pumps are made for mothers who have been home breastfeeding a healthy term infant for at least 6 weeks, and then breastfeeds at all feeds when home from work and on days off. I find many mothers use the working mom type pumps (while they are excellent pumps for what they are intended for) when they should be using the hospital grade types, and they lose their supply pretty quickly.
I'm sure there have been mothers in the above categories who have maintained supply with the working mom type pumps, but it is a risk in my opinion.
