On the other hand, I had a call on our hospital "help-line" the other day in
which the mother was asking about treating tongue-tie now that the baby was
one week old and not gaining weight.  Her pediatrician dismissed her
concerns because he said "If the tongue-tie were affecting breastfeeding,
the LC in the hospital would have noticed it and let me know."  So there are
some Pediatricians who expect the LC to "diagnose" tight frenulums!
