I agree with Katherine, the asymmetrical latch with folding the nipple back
is not just for flat nipples.
I saw a video of this latch and I have finally been able to try it at work.
I find that if a mother uses either the cross cradle hold or foot ball hold is
better than the traditional cradle for this latch.  I have also found it
easier with mothers who do not have real large breast but that is for me to work
out.
I have the mother hold the baby in the clutch hold then with the free hand,
start on her ribs and slide her hand up under the breast.  Then she places her
thumb near the nipple and pulls it back slightly.  Her thumb placement should
be perpendicular to the baby's top lip.  Now bring the baby's chin to the
areola at the point where the pulled back nipple is touching the baby's nose.  The
baby roots and opens very wide to reach the nipple.  Once the baby reaches
the nipple to let it go into the baby's mouth.  The chin and bottom lip are
already planted well below the nipple so you have the asymmetrical latch.
I have used this on moms with sore nipples and it has 2 benefits.  The
asymmetrical latch helps to decrease pain and with the mother pulling the nipple
away from the baby's mouth until they have this very wide open latch makes them
less gun shy.
