RE:  Patient denies......

Many nurses (myself included) were taught this as the proper way to chart
and communicate with others about a patient. It simply means...I asked
about a certain symptom, allergy, history or whatever...the patient said
no.  It is a simple way for nurses to communicate without saying or writing
each time, I asked "..." & she said "...", everytime we document a symptom
etc. There are no value judgements attached to it.
