How about:
1. Did you receive any medications?
2. Did you ask for/take anything for pain?
3. Do you know if you received any shots or pills or medication in your
intravenous for pain, sleep, other?
This would include Demerol, etc.
4. Did you have any type of anesthesia, block, epidural or were you put to
sleep?
5. Do you know if you had a local anesthetic (*most won't have any idea of
this)

Keep it simple and use terms that people (as opposed to medicals) can relate
to as many people don't know what was given to them. For any kind of research
that needs to be an accurate reflection, one would need to look at the
charts.
