Hi all, The following is from a colleague of mine who is looking for help.
I've looked in the archives under shooting pains and candidias and got a few
posts, however it doesn't sound like this is simply yeast....

Mom of  8 day old complaining of shooting pains up to armpit during first
minute of feed, then easing, then tingling/burning.  Mom mostly feels this
on the L breast, but has also felt it on the R.  In between feeds, mom
described both an achy/burning sensation and pins & needles in breast
tissue.  Mom's L nipple has a scar across the top from about the 10-2
o'clock position.  This is from breastfeeding her firstborn (now 2 yrs.
old).  Mom stated that with firstborn, she developed a crack on her L nipple
starting the 2nd day.  The crack remained open throughout 11 months of
breastfeeding and for an additional 2 months after ending (bfg.).  During
that time she consulted with LLLL, LC, pediatrician to help; she tried
pumping instead of bfg., lansinoh, treatment for yeast, breast shields, air
drying, checking latch/position.

Have been talking with mom since middle of her 3rd trimester.  At first
visit, 5 hrs. postpartum, she mentioned (after feed) R side sore on nipple
and tender all around breast.  Did not observe latch on L side at this time.
(Mom is R-handed, large breasted.)  Second visit was about 20 hrs. later,
mom observed baby has stronger suck/latch than older sib had.  No observed
latch.  Third visit, 5 days postpartum.  Mom described soreness in both
nipples and pain in both breasts.  Pain described as burning thru breast,
hurts most at beg. of feed, then diminishes.  Observed latch on both L & R
sides.  All looked as should for both mom and baby.  Only adjustment was to
have mom center nipple in baby's mouth.  Mom said that felt better.  Agreed
to f/u in 2-3 days, or call if problems.  F/U today, mom mentioned above
pains.  As of my visit/observations on Mon., there were no signs of cracks,
blisters, bleeding.  Only observation was small, about 1/8 inch (3-4 mm?)
brown spot on each nipple.  Mom also states baby's latch not as strong as at
first.

Birth was induced, pitocin only med.  (No epidural, etc.)  No forceps or
vacuum.

Any help greatly appreciated.
