Hi,
I would look for an additional diagnosis along with the oversupply that may
account for her pain.  The fullness alone at 20 days is not unusual, but the
scabbed nipples and deep pain are very unusual.  I would assess for a
topical infection or early mastitis (treatment with Lotrisone and Bactroban
topicals or with Antibiotic/antifungal oral meds).  Also you may find that
there  is continued latch injury that may require a period of complete
nipple rest for 48 hours or more (use a good quality pump and pain relief)
or there may be underlying Raynauds which may not be able to be diagnosed
until the nipples are healed.
