Once milk volume increases, mothers need to switch from the preemie mode to standard mode.  The reason is that once there is enough volume, the pump's job is to get the milk out as fast as possible.  The pauses in the preemie mode are not conducive to that once there is sufficient volume, thus causing a "back-up" of milk and engorgement.  What she might also be noticing is more rapid increase in milk volume because of using the preemie pump.

Subject: Engorgement with Symphony Preemie Setting?

A staff nurse I work with told me today she is seeing more engorgement with mothers using the preemie setting on the symphony pump, on 2nd-3rd day post partum..
